Midgard's the planet, Rune-Midgard's the country.
I always considered Lutia to be member of the "Island countries" because it's not attached to the mainland and you need to reach it via boats.

Considering the popular lore with Santa clause was that he lives at the North Pole, I thought Lutie was at the north pole, with the rest of the Lutia lands bordering it.
I think it's a bit difficulty to understand the geopolitics divisions of the Ragnarok world by just playing. There's not much information in-game.
But after seeing
this page in irowiki, it all made sense. Specifically the table in bottom page: 3 countries, a place in another dimension (in time and space?), city-states, towns representing real world locations.
